POBL A GWASANAETHAU A ALL HELPU

Mae gan bob consortiwm addysg rhanbarthol brif gydlynydd Grant Datblygu Disgyblion ar gyfer plant sy’n derbyn gofal (PDG-LAC). Mae’r prif gydlynydd PDG-LAC yn gyfrifol am ddefnyddio’r cyllid grant mewn ffordd strategol er mwyn helpu plant sy’n derbyn gofal a phlant wedi’u mabwysiadu yn eu rhanbarth. Yn ogystal, mae gan bob awdurdod lleol gydlynydd Addysg Plant sy’n Derbyn Gofal (LACE). Pryd bynnag y bydd unrhyw beth yn eich poeni chi am blentyn sy’n derbyn gofal gennych neu os ydych chi eisiau cael gwybod am rywbeth, cysylltwch â’r cydlynydd LACE ar gyfer eich awdurdod lleol. Os na fyddant yn gallu eich helpu chi eu hunain, byddant yn gallu eich cyfeirio at rywun a all helpu.
